What happened?
On or about January 25, 2025, Northwest Radiologists encountered a network disruption. They quickly initiated an investigation and identified that certain data on their network was improperly accessed between January 20, 2025, and January 25, 2025. The potentially affected information may have included personal details such as your first and last names combined with some of the following: address, phone number, date of birth, email, Social Security number, driver's license or state ID number, medical treatment or diagnosis information, provider name, medical record or patient ID, health insurance details, or treatment costs. Up to 350,000 individuals have been affected by this data breach.
